4. We will keep the individual and their family actively involved throughout the process to ensure the chosen home meets their needs.
5. We will then assist landlords in making necessary adaptations for a comfortable living environment once a home is selected.
6. Afterwards, we will continue to liaise with landlords after the home is found to ensure ongoing suitability and address any further needs, aiming to create a thriving, independent living environment for the individuals supported.
